Recently many AdSense accounts has been disabled with the mention of Invalid Click Activity or Policy Violations eventhough the AdSense user has no responsible over it. But Google doesn't care them.
If it's claimed due to Invalid Click Activity, you may appeal from here ~ http://www.google.com/support/adsense/bin/request.py?contact=invalid_clicks_appeal

ya i had once been disabled.
Try applying again with a new gmail id and hope they approve you!

yaa. I got a mail after i closed my adsense account that i won't be able to reactivate my adsense account but will have to start from another gmail account.
So i created another account and applied for the same blog. It's under review now.

Google is so brutal. I'm disabled too for using a traffic building site. How lame. Try Amazon for affilliate material. They have a good program that isn't so strict. Personally, I think Google is using the blog community to increase there bottom, or cover the costs of creating Blogger. Seems almost illegal what they do.

you dont need to create a new Google account. Today you can reapply to adsense even it was banned. Mine was banned many times and applied many times, then I finally got adsense after 5 times of resubmitting, if it was not accepted you can read the reasons, why, and try repairing your blog to be adsense ready

Nope not yet but I may experience it soon.
I had clicked my own ads unknowingly at the time I was newly approved for ad-sense.
But they have finalized amount of that time and after that, I never clicked my own ad.
Lets hope for best ;)

for you, if you are creating another gmail ID for applying again for adsense, its OK.
but I don't believe that there are chances that they would approve application if you have given the same shipping address the one you have given in your previous banned adsense account.
Because they check addresses while registering new account.
It happened with me once. So, I would suggest you to choose another shipping address of any of your relative for new adsense account.
But still try it once with old address if they approve, its good... else try different address .. :)
